Telehealth appointments are useful for out-of-town patients, or patients desiring to gain more information prior to an in-person office visit.
Pay
Payment is due prior to the appointment. We accept Zelle ($49 to coberhealth@gmail.com) or credit/debit card (see button). Note there is no payment required for Medicare / Medicare Advantage PPO patients (see below).
Complete Intake
Patients with Medicare or Medicare PPO must complete our intake prior to the appointment as we do not collect fees to stay compliant with Medicare regulations. For all other patients, completing our intake form prior maximizes the time in your consultation to answer your questions, and allows us to request medical records on your behalf prior to the appointment.
Please note that failure to complete payment and/or intake completion prior to the appointment day can result in the cancellation of your appointment.
