Self-pay Patients

For self-pay and with certain insurance plans, we require the out-of-pocket responsibility to be paid prior to services being rendered. To receive a refund for any cancellations we require at least 24 hours' notice.

All prices posted and quoted are cash-prices. Payments can be made by the following methods: 

1. Check payable to "Meshach Cober MD" which may be mailed to: 658 W Indiantown Rd, Suite 212, Jupiter, FL 33458 (check must clear prior to service)

2.  Zelle (recommended): coberhealth@gmail.com

3. Credit/debit card: via Square payment buttons
   Our credit card prices are 4% higher than our cash prices and are eligible for 96% refunds.

4. Financing via Cherry:
   Our financing prices may be higher than our cash prices.
